IKF Minor Release forms

• An IKF minor release form must be completed by the minor's Parent or Legal Guardian before the minor can participate in a PKRA race. A form must be completed at each race UNLESS a form covering the calendar year is on file with PKRA.
   
• Beginning in 2012, IKF requires on-file minor release forms to be notarized. The document must be signed by the Parent or Legal Guardian. Their signature must be notarized. Please remember a legal guardian is not the adult a minor may be with at the event that day.
• To assist with notarizing on-file forms, PKRA will have forms and two Notaries at Registration for the first two races. There will be no charge to PKRA members for this service. Non Members will have to pay a $5 fee if they wish to have the form notarized and on record with IKF for 2012.
• IKF minor release forms can also be notarized by any Notary. Blank forms can be obtained at any race. But until a notarized minor release form is on file, a form must be completed for each race. 
• Please bring a valid form (i.e. driver’s license) of Identification for the Notary.

There are NO changes regarding minor release forms for practice sessions.
